Will the background cover my text or images?

No. The color is inserted underneath the existing page content as a real background layer, so everything on the page remains fully visible.

How do I choose a color?

Enter a standard hex value — #FFFFFF is white, #FFF8E7 is a soft sepia, #F5F5F5 is light grey. Both #RGB and #RRGGBB formats work.

Does it work on scanned PDFs?

Scanned pages are full-page images that already cover the background, so the new color only shows in unpainted margins, if any.
